Privacy Policy
Atlantic Smiles Dental LLC ("we", "us", "our") operates atlanticsmilesdental.net. This policy explains what information we collect through this website, why we collect it and what we do with it. It covers the website only. Protected health information created or received in the course of your dental care is governed by our HIPAA Notice of Privacy Practices.
1. Information we collect
Information you give us
- Appointment requests: your name, telephone number, email address, whether you are a new or existing patient, the treatment you are asking about, your preferred date and time, and any details you choose to type into the message field.
- Online payment requests: your name, telephone number, email address, the item you are paying for and an optional reference. We do not ask for and cannot accept card numbers, expiry dates or security codes through this website.
- Correspondence: anything you send us by email or telephone.
Information collected automatically
Our web server records standard technical information when a page is requested, including the IP address, the time of the request, the page requested and the browser type. This information is used to keep the site working and to protect it against abuse. We do not use it to build advertising profiles, and we do not sell it.
2. How we use the information
- To contact you about your appointment request and confirm a time;
- To process and confirm a payment you have asked to make;
- To answer questions you send to the office;
- To keep records required by California dental and business regulations;
- To keep the website secure and working.
We do not sell your personal information. We do not share it with advertising networks.
3. Who we share it with
- Service providers that host the website and deliver the forms on it, and that process a payment you authorise. These providers act on our instructions and are only given what they need to perform that task.
- Dental laboratories, specialists and insurance carriers when that is necessary to provide the treatment you have asked for.
- Legal and regulatory recipients where we are required to disclose information by law, court order or a lawful request from a public authority.
4. Cookies and similar technologies
This website does not use advertising or tracking cookies. Your browser may store small amounts of data locally so the site displays correctly. You can block or clear this in your browser settings without losing access to any part of the site.
5. How long we keep information
Appointment requests are kept for as long as needed to arrange and follow up on your care. Dental records are kept for the period required by California law. Technical server logs are kept for a short period and then deleted.
6. Security
The website is served over an encrypted HTTPS connection. Access to appointment and payment request data is limited to staff who need it to do their job. No method of transmission over the internet is completely secure, so please do not send sensitive medical details through the website forms — tell us those in person or by telephone.
7. Your California privacy rights
Under the California Consumer Privacy Act, as amended, California residents may request:
- the categories and specific pieces of personal information we hold about them;
- the categories of sources and the purposes for which it was collected;
- deletion of personal information, subject to the records we are legally required to keep;
- correction of inaccurate personal information;
- to opt out of any sale or sharing of personal information — we do not sell or share personal information, so there is nothing to opt out of.
To make a request, email atlantic@atlanticsmilesdental.net or call (323) 554-0072. We will verify your identity before acting on the request and will respond within the timeframe the law allows. You will not be treated differently for exercising these rights.
8. Children
This website is not directed at children under 13 and we do not knowingly collect personal information from them through it. Appointment requests for a child should be submitted by a parent or legal guardian.
9. Changes to this policy
If we change this policy we will post the new version on this page and update the date at the top.
